80% SPECTACLE REPLACEMENT WARRANTY
- This offer is subject to the purchase of a spectacle frame (excludes grey, brown and orange colour coded frames).
- Colour coded frames and prescription lenses will be covered for 80% of the original cost and excludes the price of the eye test and promotional discounts.
- The 80% Spectacle Replacement Warranty does not include cover for Glass lenses. Lenses in a rimless frame are only covered if fitted with Poly carbonate or Trivex lenses.
- The 80% Spectacle Replacement Warranty is only valid on the 1st claim and applies to colour coded frames and prescription lenses only.
- The patient has to co-pay the 20% in CASH on the 1st claim.
- Should your new spectacles exceed this amount, you will have to pay the difference on your own account.
- The 2nd claim will be covered for 50% of the original cost on colour coded frames and prescription lenses and excludes the price of an eye test and promotional discounts.
- Spectacle Replacement Warranty is valid from the date of purchase for 2 years.
- Only claims for accidental loss or damage can be submitted for processing.
- Claims are only valid provided that the patient uses his / her same script at the date of the original purchase.
- This Spectacle Replacement Warranty is limited to 2 claims within the benefit period.
